Where the Data Comes From
T-MedInfo is based on official NPI Registry data, Medicare Part B utilization data, Medicare Part D prescribing data, Sunshine Act Open Payments disclosures and Medicare enrollment (PECOS) records. An NPI record shows registration in NPPES; it is not a state license and does not show that a provider currently practices.
US State Healthcare Density Leaderboard
Comparative per-capita practitioner concentration across all 50 US states & territories
Across the United States, NPPES lists 7,411,289 active individual NPI records (Type 1, individual practitioners) and 1,958,206 active organization NPI records (Type 2: hospitals, clinics, pharmacies, labs, suppliers and other organizations) with a US practice address. In the 50 states and DC, with a population of 334,922,499 residents (ACS 5-year 2020-2024), that is 2,198 individual NPI records per 100,000 residents. An NPI record shows registration in NPPES; it is not a state license and does not show that a provider currently practices.
Of the states with population data, District of Columbia has the highest rate, 6,934 practitioners per 100,000 residents, and Puerto Rico the lowest, 1,265 per 100,000.
High-density metropolitan corridors support extensive medical subspecialization, while rural states increasingly leverage primary care networks and virtual consultation options to meet healthcare demand.

National Healthcare & Financial Disclosures Dashboard
Aggregated from CMS Part D Prescribers, Open Payments, Medicare (PECOS) enrollment and Part B data
This national dashboard aggregates public datasets published by the Centers for Medicare & Medicaid Services (CMS). It integrates Medicare Part D prescription claims, Sunshine Act Open Payments financial disclosures, PECOS provider enrollments and Medicare Part B allowed procedure volumes across U.S. states and territories.
In the CMS Part D Prescribers data for 2024, Part D prescriptions account for $226.7 Billion in total drug cost. 2,540,932 practitioners have a Medicare (PECOS) enrollment record. Open Payments data for program year 2025 list $3.1 Billion in payments reported by drug and medical device companies; a reported payment does not necessarily mean the financial relationship is improper.

Understanding NPI Registration & Enumeration Data
What is NPI Enumeration?
In the US healthcare system, "enumeration" is the official process where CMS assigns a unique 10-digit National Provider Identifier (NPI) to individual practitioners (Type 1) and healthcare facilities (Type 2).
Why the Peak in 2005–2007?
When CMS launched the NPI system under HIPAA federal law in May 2005, millions of existing US physicians and facilities registered simultaneously to meet the federal compliance deadline in May 2007.
Annual Trends (2008–2026)
Each later year counts the NPIs CMS assigned for the first time that year, to new individual practitioners and to new organizations or practice locations that applied for their own NPI. A new NPI is a registration event; it does not show when a license was issued.
